Reminder - Year 6 Transition to Secondary School resource pack for all pupils (sent 4th May)
Transition Support Programmes is part of the Adolescent Development Services. It delivers targeted development programmes to young people in primary school who require additional support to overcome challenges to their personal, educational, health, and social progression in the transition to secondary school. In September 2019, Transition Support Programmes rolled out the SWITCH 15-month pilot programme to referred groups of young people at six individual primary schools in the borough. This programme consists of:
- a 24-week group work intervention to identified groups of young people from each of the primary schools
- an additional 12-week further group work support programme to identified young people
- one-to-one intervention alongside the further group work support detailed above which continues over the summer break
- access to a summer activity programme for all SWITCH attendees
- 12-week further one-to-one support to identified young people from the September for their first term at secondary school.
The Transition Support Programmes 'Moving to Secondary School' Resource Pack consists of a variety of new and tried and tested resources that Transition Support Programmes use in their interventions. It was created with a view to continue to provide support to all current Year 6 SWITCH attendees since the programme was called to a halt due to the current pandemic.
It's expected that all year 6 pupils find this useful to support transition.
