Early Identification Toolkit

The reviewed Early Identification Toolkit is now available and linked below . It provides clear, age-specific checklists to help professionals identify emerging needs across four key areas: Communication & Interaction, Cognition & Learning, Social, Emotional & Mental Health (SEMH), and Sensory & Physical.

The toolkit has been co-produced with a wide range of local professionals to ensure it is practical, consistent, and relevant across different settings. It is designed to support early conversations, shared decision-making, and timely action:

 

Checklists have been broken down below into Need Type. If the child you are working with does not meet the criteria stated in the identified age range, please refer to an earlier age range.
