It provides clear, age-specific checklists to help professionals identify emerging needs across four key areas: Communication & Interaction, Cognition & Learning, Social, Emotional & Mental Health (SEMH), and Sensory & Physical.
The toolkit has been co-produced with a wide range of local professionals to ensure it is practical, consistent, and relevant across different settings. It is designed to support early conversations, shared decision-making, and timely action:
